We offer one of the most comprehensive transplant programs in the world and perform a variety of transplantations. A caregiver in this position works 8:00am-5:00pm. A caregiver who excels in this role will: Complete psychosocial assessment for selection of a candidate for organ transplant and living donor. Conduct psychosocial assessment and interventions; makes treatment recommendations and engages patients/families in adherence to the treatment plan by reducing or eliminating, to the extent possible, the social, psychological, financial, behavioral, and regulatory barriers to successful medical outcomes. Communicate with interdisciplinary team regarding the discharge planning status of all patients referred by them. Educate patients/family and physician regarding post-acute options and address freedom of choice and financial concerns. Participates in discharge planning for assigned patients to ensure a timely discharge and provide appropriate linkage with post-discharge care providers and resources. Serve as a resource person and provides counseling and intervention related to treatment decisions and end-of-life issues. Advocate for patience and family. Provide intervention in cases involving child abuse/neglect, domestic violence, elderly abuse, institutional abuse, and sexual assault. Advocate for patient and family empowerment and independence to make autonomous health care decisions and access needed services within the health care system. Minimum qualifications for the ideal future caregiver include: Master's degree in social work or Master's of Science in Social Administration (MSSA). Current state licensure as a Social Worker LISW or LISW-S. Exposure and experience in pre-acute and post-acute care. Excellent communication and negotiation skills necessary to work collaboratively with multidisciplinary teams for optimal outcomes. Show ability to work independently and with teams. Must be flexible, open-minded, and adaptable to change. Preferred qualifications for the ideal future caregiver include: Hospital Social Work experience.
