Job Summary
  • Under the direction of the program Director, the transitional care specialist is accountable for facilitating, implementing, and confirming postacute transition plans for CoxHealth s patient population. The position will respond to hospital and community-based service requests from the nursing staff, case managers, social service workers, hospitalists, or private attending physicians' practices.
Job Requirements
  • Education
    • Required: High school diploma or equivalent
    • Preferred: Associate's Degree
  • Experience
    • Preferred: One year hospital and/or healthcare experience
  • Skills
    • Good communication skills Ability to communicate effectively in written format and/or oral presentations
    • Strong analytical & problem solving skills; semi-independent in decision making
    • Ability to maintain organization in a changing environment
    • Exhibit initiative, responsibility & flexibility
    • Must be able to initiate and understand research related to projects
    • Learn and appropriately utilize all required computer applications, including but not limited to E-mail, Windows-based or Microsoft programs
    • Ability to write effective documentation for processes and procedures.
    • Ability to make routine decisions in accordance with departmental policies and procedures.
