Supportive Services Case Manager

LHH Recruitment Solutions is hiring a Supportive Service Case Manager for our nonprofit client located in the Santa Ana area. This role will be 100% onsite, contract-to-hire, and a swing shift schedule. This schedule will be from 3p-11:30p and interested candidates must be available to work at least one weekend day.

This role will be focused on supporting our client's program participants in their interim housing site and help upon the intake with behavioral and primary healthcare, getting documentation, helping with insurance needs, and other supportive services in order for participants to move forward towards a more permanent housing solution. This position will be working with the unhoused population, including individuals who are experiencing mental health and/or substance abuse ailments.

Requirements
  • High School Diploma or equivalent
  • Minimum 2 years case management/social services experience
  • Proficient working within HMIS and Microsoft Office
Skills
  • Compassion and empathy towards others while being able to maintain a professional boundary
  • Strong written and verbal communication and ability to speak to a wide array of individuals
  • Can de-escalate high-pressure situations in a calm demeanor in order to find the best outcome
  • Excellent multitasking ability and prioritize and adapt to different tasks throughout the day
  • Able to work well within multiple software systems

Pay Details: $23.00 per hour
