Your Role: Supporting Recovery

As a Supportive Housing Case Manager, you'll guide individuals with Substance Use Disorders (SUD) toward achieving sustained recovery and self-sufficiency. Your work will ensure that our clients feel supported, empowered, and connected to the resources they need to thrive.

What You'll Achieve
  • Empower Clients: Develop individualized service plans and connect clients to support systems, community resources, and employment opportunities.
  • Ensure Safety and Standards: Maintain housing facilities to meet GAAMHA's high standards for safety, hygiene, and appearance.
  • Foster Accountability: Administer urinalysis and breathalyzer tests to ensure compliance with program rules.
  • Build Connections: Facilitate weekly house meetings and work closely with community partners to address client needs.
  • Provide Support: Coordinate transportation for clients to medical appointments, court appearances, and more.
  • Promote Excellence: Maintain accurate records, prepare reports, and adhere to confidentiality regulations.
  • Represent GAAMHA: Serve as a community ambassador, networking and promoting our values and professional standards.
What Sets You Apart
  • A Bachelor's Degree or equivalent experience in a related field.
  • CADC or LADC certification and residential treatment experience are preferred.
  • Familiarity with 12-Step and other mutual aid support programs.
  • Strong oral, written, and interpersonal communication skills.
  • Cultural competence and compliance with HIPAA and 42CFR Part 2 standards.
  • Proficiency in MS Office and the ability to manage documentation effectively.
  • A valid driver's license and commitment to providing client-centered care.
Who You'll Lead and Support
  • Supervise: House managers across assigned supportive housing facilities.
  • Collaborate With: The Director of Supportive Housing for administrative supervision and the Clinical Supervisor for clinical oversight.
