Mercy Haven currently has an opening for a Supported Housing Coordinator. In this role, you will assist families and those surviving domestic violence in securing and maintaining entitlements. You will work with the Social Service Housing Unit to ensure families maintain housing and become self-sufficient.

POSITION RESPONSIBILITIES
  • Assist residents in securing and maintaining entitlements (e.g., SSI/SSD, PA Medicaid, SNAP, Section 8) and provide referrals for unmet service needs.
  • Support residents in developing and implementing long-range service plans in collaboration with the supervisor and staff.
  • Conduct monthly home visits and phone contacts, providing additional crisis support as needed, with appropriate documentation.
  • Maintain accurate and current records, including resident files, phone logs, and documentation of resident interactions and collateral service provider contacts.
  • Notify the supervisor of crisis situations and document incidents accordingly.
  • Assist with resident move-ins and move-outs, ensuring living spaces are clean and prepared.
  • Responsible for contact and documentation of such with collateral service providers at least once a month and as needed.
  • Facilitate parent support group based on participant needs such as employment skills, education, vocational work, and life skills training.
  • Participate in staff meetings, supervision, training, and interagency collaborations.
  • Conduct monthly rent reviews with the supervisor and assist in discharge planning.
  • Ensure program compliance with record-keeping, site conditions, and funding requirements.
  • Conduct outreach to local DSS and shelters and Housing Support Agencies, Hud-Vash (Section 8) to assist in program occupancy levels
  • Providing quarterly documentation requirements for NYSSHP Grant funding
  • Involved in the internal and external process of Homeless Housing Applications; processes referrals and completes screenings.
  • Rotate on-call duties and perform additional related responsibilities as needed.
POSITION RELATIONSHIPS

Reports to: Associate Director of Supportive Housing

Supervises: Community Services Specialists, Wellness Specialist, LPN, Interns

POSITION QUALIFICATIONS
  • Master's Degree in Social Work or related Human Services field preferred OR
  • Bachelor's Degree in Social Work or related Human Services field PLUS related experience required
  • CASAC and SIFI certification preferred
  • Supervisory Experience Required
  • Experience with people surviving domestic violence preferred
  • Valid NYS Driver's License with a safe driving record, access to a personal and reliable vehicle
  • Physical Requirements: Sits, stands, stoops, squats, walks, and moves intermittently during work hours, may push or pull 10-15 pounds daily, may push or pull up to 50 pounds occasionally, drive agency vehicle
Training will be completed Monday-Friday, 8:30am - 4:30pm

SCHEDULE: Tuesday - Saturday, 8:30am - 4:30pm

PAY RATE: $55,400 per year ($28.41 hourly)

LOCATION: Main Office in Islip Terrace, NY
