Unity House of Troy's Supported Housing Program is seeking a community minded, compassionate individual to assist people living mental illness to live independently in the community.
Responsibilities:
Provide housing support services to individuals living with mental illness.
Teach independent living skills to help people live in the community in the level of their choosing.
Make referrals and advocate for consumers to link participants to entitlements and supports.
Work collaboratively with other case managers and providers to wrap around services.
Practice a harm reduction philosophy.
Promote a culture of inclusion and belonging.
Requirements:
High School diploma or GED required. At least 1-year previous experience working with at risk-populations required. At least 1-year experience working with those living with mental illness, substance abuse, or in the human services field. Must have reliable transportation as you will need to travel for this position. Must have a valid and clean New York State driver's license. Must be able to work independently.
