General Summary:

The Critical Care Outreach Team (CCOT) Supervising Nurse is responsible for leading, developing and coordinating the Critical Care Outreach Team, and provides immediate and efficient care for patients experiencing acute clinical deterioration during and outside Rapid Response events. This role involves overseeing team operations, ensuring high standards of patient care, and collaborating with other healthcare professionals to improve patient outcomes and safety. This individual will serve as a clinical expert and work closely with critical care teams to execute appropriate, timely interventions.

Responsibilities:

1. Leadership and Coordination: a. Lead and manage the Critical Care Outreach Team (CCOT), ensuring prompt and effective response to medical emergencies within the hospital. b. Coordinate with various departments to facilitate timely and appropriate patient care, including safe transfers to higher levels of care, as appropriate. c. Mentor and support team members, fostering a collaborative and professional environment. d. Collaborate with senior leadership to establish and update policies, procedures, and process improvement strategies.
2. Clinical Expertise: a. Provide expert clinical care and decision-making for patients experiencing acute deterioration. b. Perform rapid assessments, initiate appropriate interventions, and monitor patient status. c. Utilize advanced clinical skills to manage complex and critical situations. d. Provide available and actionable consultation for at-risk patients on inpatient units.
3. Communication and Collaboration: a. Serve as a primary point of contact during rapid response events, ensuring clear and effective communication with patients, families, and healthcare providers. b. Collaborate with physicians, nursing staff, and other healthcare professionals to develop and implement patient care plans. c. Participate in interdisciplinary meetings, and contribute to case discussions and debriefings.
4. Quality Improvement: a. Monitor and evaluate the performance of the Critical Care Outreach Team (CCOT), identifying areas for improvement. b. Implement evidence-based practices and protocols to enhance patient care and outcomes. c. Participate in quality improvement initiatives and projects, collecting and analyzing data to inform practice changes and promote fiscal responsibility.
5. Education and Training: a. Provide ongoing education and training for Critical Care Outreach Team (CCOT) members and other hospital staff. b. Conduct simulation exercises and drills to maintain team readiness and competence. c. Promote and enforce current best practices, clinical guidelines, and advancements in emergency and critical care nursing.
6. All other duties as assigned within the scope and range of job responsibilities.

Education: Master Degree

Experience:

Required: 5 years of clinical nursing experience, with at least 3 years in a critical care setting.

Preferred: 7 years of clinical nursing experience, with at least 5 years of clinical nursing in a critical care setting. Previous leadership experience as an educator and/or Clinical Coordinator in a critical care setting
