We're currently seeking a dedicated Registered Nurse - After-Hours to join our team. This pivotal role provides evening and overnight support for home health patients, helping ensure peace of mind and continuity of care during non-business hours. As an After-Hours Registered Nurse, you will be responsible for assessing, coordinating, and delivering patient care during evenings, nights, and weekends. Your role includes:

• Performing initial assessments and follow-up evaluations for a home health patient caseload.

• Identifying physical, emotional, and environmental care needs and developing safety-focused care plans.

• Communicating critical updates to the clinical supervisor, physicians, and care teams.

• Responding to changes in patient condition and initiating appropriate nursing interventions.

• Implementing and verifying physician orders in line with individualized care plans.

• Maintaining accurate and timely clinical documentation.

• Participating in case conferences and collaborating to ensure continuity and quality of care.

• Performing other duties and special projects as assigned.

• Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Bachelor's)

• Current RN license in the state of practice

• At least 1 year of clinical nursing experience (home health preferred)

• Willingness and ability to travel approximately 50%

• Strong critical thinking, communication, and documentation skills

• Compassion, reliability, and a patient-first mindset
