What You'll Do:
- Provide home health visits, admissions, and assessments for skilled nursing care: wound care, med administration, disease management
- Develop and follow individualized care plans in collaboration with physicians
- Monitor and document patient conditions via electronic tablet charting
- Serve as case manager and care coordinator for your clients
- Build meaningful relationships with clients and their families
What You'll Need:
- Active Massachusetts RN license
- A passion for home health care and helping people live independently
