Job Summary and Qualifications The Clinical Nurse Coordinator (CNC) ensures and delivers high quality, patient-centered care and coordination of all functions in the unit/department during the designated shift. In collaboration with other with established policies/procedures, serves as a resource person for staff, and models a commitment to the organization's vision/mission/values to support an unparalleled patient experience and clinical outcomes that contribute to overall departmental performance. Job Functions:
• Collaborates with a multidisciplinary team to coordinate, evaluate, customize, and deliver high quality, patient-centered care.
• Reflects core values when interacting with co-workers, patients, family members, and others in the community to foster an environment where employees are engaged and where patient care is unparalleled.

• Completes other duties as assigned. About the unit:
• 14 bed Transplant ICU
• Patient to nurse ration 2:1, depending on acuity may need 1:1
• Patient population: ESRD, ESLD, Solid abdominal organ transplant pts (Kidney deceased and living, deceased liver, and pancreas transplants)
• Unit offers individualized orientation with dedicated preceptors and specialized classes and trainings
• Doctor involvement with bedside grand rounds and opportunities for education for the staff from the physicians and providers.
• Our nursing team is dedicated to maintaining a safe patient work environment and welcome new nurses.
• We are a team-based unit with a family like culture and inclusion. We enjoy teaching moments and opportunities for any level of nursing experience.
• Nurse leader and educator rounding to ensure all staff have what they need and are receiving the most out of their orientation, training, and nursing experience.
• Quarterly and as needed staff meetings for updates, changes, progression, and challenges. We like to have fun and will play games at staff meetings.
• Unit based council for the staff to have their concerns and voices heard and to improve our patient centered care.

• Graduate of an accredited diploma, associate or baccalaureate degree in nursing required
• BSN preferred
• Minimum of 1 year working as a Registered Nurse (RN) required
• 3 years of RN experience in an ICU setting preferred
