Job Description:
Help Build Something Meaningful With Us As a Behavioral Health RN Case Manager in the Palmer Territory with Elara Caring, you will provide compassionate, individualized care to behavioral health patients in their homes. You will manage a dedicated caseload, conduct assessments, develop care plans, and coordinate interdisciplinary care, helping patients achieve better health outcomes in the comfort of their own environment.
Key Responsibilities:
Assess the physical, psychosocial, and environmental needs of behavioral health patients and families
Develop, implement, and evaluate individualized treatment plans
Conduct initial and follow-up nursing assessments
Communicate critical patient updates to supervisors, physicians, and care teams
Provide education and counseling to patients, families, and caregivers
Teach behavioral health concepts and symptom management to staff in various care settings
Supervise LPNs and behavioral health aides in care delivery
Provide support and conduct bereavement assessments at the time of death
Qualifications:
Current, unrestricted RN license
1+ years of clinical nursing experience
Reliable transportation, valid driver's license, current auto insurance
Willingness to travel 50% within the Palmer Territory
Associate or Bachelor's degree in Nursing (preferred)
Previous Home Health or Hospice experience (preferred)
Familiarity with Homecare HomeBase (preferred)
