Key ResponsibilitiesDirect and oversee home health services delivery, ensuring diagnosis-specific patient care
Manage patient care services and personnel, including assignments, referrals, assessments, and care plan updates
Coordinate services with providers, vendors, and facilities
Ensure timely initiation of patient care per regulatory guidelines
Oversee physician orders, staff assignments, and team communication
Review and approve care plans and integrate physician orders
