Entitlement Specialist

JOB SUMMARY
The Entitlement Specialist will be supervised by the Managing Directors of Clinical and Social Services. The Entitlement Specialist will be responsible for helping clients obtain and maintain the following core five benefits: Medicare Part D Extra Help (Low Income Subsidy, LIS), Medicare Savings Program, SNAP, Low- Income Home Energy Assistance program (LIHEAP) and Medicaid.
JOB DUTIES & RESPONSIBILITES
Responsibilities include but are not limited to:
- Submit new enrollment, or renewal applications with, or on behalf of clients from among the five core benefits.
- Participate in training on BenefitsCheckUp and other onboarding required trainings to promote successful grants management.
- Participate in a minimum of one capacity building/training session per quarter during the grant cycle.
- Attend Age + Action Conference.
- Escort clients to file applications as necessary.
- Monitor benefit application process, resolving problems and advocating for clients as needed.
- Work with client/family/client representative to gather any additional documentation required to finalize benefit applications.
- Work with clients to address any issues that arise, such as inactive Medicaid numbers.
- Meet with workers at local Medicaid and SNAP offices to help resolve problems when necessary.
- Provide timely and accurate information to Social Workers/Case Managers regarding status of each application and any related problems.
- Submit monthly reports on all relevant metrics in a complete and timely manner.
- Other duties as required.
REQUIRED KNOWLEDGE SKILLS & ABILITIES
- Bilingual Spanish highly preferred
- Ability to speak, read and write English
- Emotionally, mentally and physically able to perform job responsibilities
- Strong computer and organizational skills
REQUIRED EDUCATION, EXPERIENCE AND CREDENTIALS
- 4-year degree from an accredited college or university in Social Work or similar discipline or high school diploma with a minimum of 5 years of experience working with the elderly, the homeless or persons challenged by mental illness.
- At least 1 year of experience working with the elderly, the homeless or persons challenged by mental illness.
PHYSICAL REQUIREMENTS
- This role may have physical demands including, extended walking and standing, bending, and pushing.
