We are hiring dynamic, passionate Speech Language Pathologists (SLPs) who want to be part of transformational change in home and community care through a truly client-centered approach. You will work in an environment that empowers the care team with professional autonomy and responsibility. You will be part of a diverse team providing services to clients in the community, focusing on holistic care planning across physical, cognitive, social, emotional, and spiritual domains. Each client has a primary care clinician leading their care planning, with interdisciplinary support to achieve client goals. As part of the SE Transitions team, you will be accountable for the care you deliver and the outcomes achieved.

Responsibilities
  1. Conduct assessments
  2. Formulate goals and recommendations
  3. Plan and implement client-centered, goal-directed treatment programs
  4. Participate in programs such as Aphasia (if applicable)

Minimum Requirements
  • Graduate of an accredited Speech Language Pathology program, in good standing with the College of Audiologists and Speech-Language Pathologists of Ontario
  • Experience in acute/rehab or community-based rehabilitation preferred
  • Reliable, flexible, creative, self-directed, and able to collaborate effectively with a high-performance team
  • Knowledge of community resources is an asset
  • Strong interpersonal, organizational, and communication skills
  • Positive work and attendance record
  • Ability to adapt to new computer applications
  • This role follows a hybrid work model, combining remote work with client visits in acute/rehab or community settings
