Position Summary
We are seeking a dedicated Social Worker to join our interdisciplinary team. The successful candidate will be responsible for providing comprehensive support to our residents and their families, ensuring high-quality care and services.
Key Responsibilities:
- Coordinate services with various healthcare providers, including dental, optometry, and other necessary referrals.
- Provide professional counseling and emotional support to residents and their families.
- Investigate, intervene, and document resident behaviors in accordance with community protocols.
- Evaluate and address residents' psychosocial needs through thorough assessments.
- Manage discharge planning processes to ensure smooth transitions for residents.
- Assist residents in accessing financial benefits they are entitled to receive.
- Maintain accurate and up-to-date Social Services records for all residents.
- Perform additional duties as assigned by management.
Qualifications
Basic Qualifications & Experience
- Bachelor of Science in Social Work or equivalent work experience.
- 1 - 2 years experience in social work, preferably in a long-term care facility.
- Experience with computer word processing and database programs.
- Must be able to read, write and speak the English language.
- Must possess excellent customer service skills
Working Conditions/Physical Requirements
- Light physical effort required by handling objects up to 20 pounds occasionally and/or up to 10 pounds frequently.
- Sits, stands, bends and moves intermittently during working hours.
- Is subject to frequent interruptions.
- Interacts with residents, family members, visitors frequently.
