Responsibilities:
- Manages referral, admission and discharge processes for program
- Develops individual Recovery Plans(IRPs) with consumers and completes additional required documentation
- Supports and assists consumers with case management duties and crisis intervention as needed
- Engages family involvement and act as a liaison between the program and other providers such as local hospitals
- Facilitates consumer appointments with APN, court system or outsider providers as needed
- Participates in 24-hour Clinical On-call rotation to provide support to staff after hours
- Provides orientation training to new employees and ongoing clinical support and direction to direct care staff
Qualifications:
- MSW in Social Work, MA in Psychology, or a related field
- Previous experience in mental health / human services
- LSW or LAC preferred
- Valid New Jersey driver's license
Shift:
Monday through Friday, 9am-5pm
