Social Worker

The Licensed Social Worker (LSW) provides exceptional psychosocial assessment, clinical intervention, crisis management, and resource development for patients with complex care needs.
•  They coordinate care across the continuum collaboratively with the healthcare team to improve patient flow, reduce unplanned visits, and support safe transitions.
•  The LSW assesses social, financial, and environmental barriers impacting patient care and engages families and physicians to align care plans with goals and system priorities.
•  They communicate effectively, resolve conflicts, and ensure patients and families receive necessary resources and education.
•  Requirements include a Master s in Social Work, active LSW licensure, healthcare or mental health experience, and knowledge of growth, legal mandates, and community resources.
•  The position is part-time, primarily days, with rotating weekends and holidays.
