ride Health is hiring a Per Diem Social Worker to support our client's medical facility based in Glendale, California. This opportunity is a great way to start working with a top-tier healthcare organization.
Responsibilities:
- Collaborate and facilitate discharge plans for patients needing post-Hospice or inpatient care.
- Assist patients and families in identifying appropriate levels of Hospice care.
- Refer patients to outpatient resources as needed.
- Initiate and reassess care plans based on patient needs and progress.
- Develop a Plan of Care (POC) that addresses psychosocial barriers, patient development stages, mental health, and family dynamics.
- Provide patient education and support throughout the care process.
- Perform other job-related duties as assigned.
Qualifications:
- Master's Degree in Social Work (MSW) in California and Hawaii
- At least 1 year of experience in an acute or long-term medical setting

- Pay Range: $65/ hour
Offered pay rate will be based on education, experience, and healthcare credentials.
