Pride Health is seeking a Social Worker to join a team in Canton GA. This is a 13 weeks contract with a possible extension.
Key Responsibilities
- Assess patients' social and emotional well-being in relation to their illness or injury.
- Evaluate medical, financial, and community resource needs to support patient recovery.
- Develop personalized care plans and coordinate community resources.
- Provide counseling and emotional support to patients and families.
- Assist physicians and healthcare teams in understanding the impact of social and emotional factors on health.
- Monitor and document patient progress, reporting changes to the Clinical Manager and physician.
- Participate in care coordination and discharge planning.
- Maintain accurate documentation in compliance with company and regulatory standards.
Patient Experience Requirements
- Experience with hospital case management and discharge planning.
Required Certifications & Skills
- Certifications: LCSW (Licensed Clinical Social Worker), BLS (Basic Life Support).
- Skills: Acute Care Hospital Experience.
