Employment Type: Full time Shift: Description: Population Health Social Worker Position Purpose: The Social Worker LSW case manages patients in their home setting to review and monitor patient engagement. Cares for patients using a team-based approach with physicians, nurses, and other members of the multidisciplinary team. Uses professional judgement for initial and ongoing assessment, planning, and evaluation of psychosocial, economic, and environmental needs of patients and families. Acts as a consultant to staff for social work service needs. This is primarily a remote position but will occasionally require a home or provider visit. Training will be onsite as well. Minimum Qualifications: Education: Bachelor's in degree in Social Work required and a master's in Social Work preferred Licensure/Certification: Licensed Social Worker by the State of Ohio Counselor and Social Worker Board Experience: Minimum experience preferred is two (2) years in social work; one (1) year in a medical setting Effective Communication Skills Current knowledge of legal, regulatory, and accrediting requirements and standards related to the practice of social work.
