We're seeking either a dedicated MSW / LMSW / LCSW Social Work Care Manager to continue the growth of our Social Work Case Management team at Emory University Hospital Midtown. As part of our team you will be responsible for coordinating clinically complex patient care needs across the health care continuum.
Our Social Worker will assess patients for transition needs including facilitating high quality clinical and cost outcomes; negotiating, procuring and coordinating services and resources needed by the patients in a timely and effective manner.
MINIMUM QUALIFICATIONS
- Must have a Masters in Social Work from an accredited Institution.
- 1 year recent healthcare experience preferred, experience in Acute Care setting preferred.
- Must have working knowledge of software/Eemr applications. ACM, CCM preferred.
- Must meet all quality and productivity expectations and successfully complete yearly competencies.
