Job Summary: Under the general direction of the Service ConnectionManager, the Service Connector provides information and referrals, community engagement and onsite activities for adults, seniors, and families living in San Francisco at our Holly Courts or Alemany Housing sites. Previous case management or supportive housing service experiences makes you an ideal candidate for this position. The Service Connector position requires a high degree of initiative, a flexible work style involving independence as well as teamwork, strong organization and follow-up, multi-tasking, and above all, interaction with a diverse group of staff, community members, and community partners. We are seeking a motivated individual who possesses a variety of skills that include the ability to develop strength-based family service plans, navigate San Francisco resources, and support service systems designed to bring sustainability to the residents.
The Organization: Bernal Heights Neighborhood Center (BHNC) is a non-profit community-based multi-service agency founded in 1978. Our mission is " to preserve and enhance the ethnic, cultural, and economic diversity of Bernal Heights and surrounding neighborhoods. We promote community action to build a just and equitable community for all. BHNC focuses on the needs of people with low and moderate incomes.
PRIMARY RESPONSIBILITIES:
Build trust with residents and be an active listener and effective problem solve of emerging issues
Conduct client intake, income verification and enroll in program to receive services
Complete assessments and identify client immediate and ongoing needs
Provide information and referral for services designed to address client needs and to create sustainability
Follow up with all client intakes within two weeks to ensure proper connections
Provide tenant engagement to include one-on-one meetings, focus groups, workshops and group activities
Conduct regular outreach to increase and improve tenant engagement through phone calls, flyering, door knocking, and emails
Attend all regular BHNC and site meetings such as: Staff meeting, tenant retention, tenant council, monthly resident meeting and resident and vendor interventions
Administrative task including updating client file updates, drafting department procedures etc.
Contribute and support workplan and playbooks associated with events and activities on site
Facilitate workshops and events
Collaborate with other non-profits, vendors and colleagues
Must be available for some evening and weekend events (create tenant activities and events to accrue at various times of the day including some morning, afternoon, evenings and weekends)
Employee must be able to travel to all program activity sites
Demonstrate ability to complete thorough, accurate and comprehensive paperwork, client documentation and reports in a timely and efficient manner.

KNOWLEDGE/SKILLS REQUIRED:
Experience conducting intakes and assessments, service plans, and post-service evaluation
Familiar with resources and support services in San Francisco
Strong organizational and communication, oral and written
Ability to multi-task and follow up and follow through on assignments
Able to exercise discretion and sound judgment on a daily basis.
Experience in complying with confidential policies and procedures that include mandated reporting laws.
Enthusiasm for leadership development and community engagement in a mixed-income, multi-racial, multi-ethnic community, including affordable housing developments.

PREFERRED QUALIFICATIONS
A minimum of 1 year of experience providing case management or service connection to adults/seniors/families
Familiar with Salesforce and/or similar system
Fluency both written and verbal in Spanish, Samoan, or Cantonese is strongly desired.
PHYSICAL REQUIREMENTS: Requires frequent sitting, typing and computer use occasionally requires pushing and/or pulling objects, walk up and down stairs and/or hills, lifting and/or carrying up to 25 lbs. requires the ability to frequently move about or remain seated depending on the task at hand.
This full-time, non-exempt position includes a competitive salary based on experience with employee paid benefits that include medical, dental, life, EAP, generous paid time off, and holidays, paid lunch, monthly cell phone stipend and more. Must be able to work 9 to 5pm Monday through Friday with occasional evening and weekend hours (limited during COVID-19). BHNC is an at will and equal opportunity employer.
