Job overview
Come join our award-winning Dementia Crisis Support Team as a Senior Band 6 Nurse (Mental Health or General Nursing). Based in Corringham, the team visits patients in their homes and care homes across Basildon, Brentwood, and Thurrock.
The team comprises experienced professionals including Psychiatrists, Nurses, support workers, and admin staff. Our goal is to reduce A&E visits and hospital admissions during crises through rapid assessment, intervention, and short-term follow-up (up to six weeks).
The service operates 7 days a week, with shifts from 9am to 7pm on weekdays and 9am to 5pm on weekends and bank holidays, on a rota basis.
Main duties of the job
Support holistic assessments of care needs for individuals presenting with diagnosed or suspected dementia in crisis, in community settings. Lead and deputize for the clinical lead when necessary, providing clinical input and leadership within the multidisciplinary team, and supporting junior staff development.
The post involves traveling across the geographical area to visit patients at home or care facilities, supporting early discharge and crisis intervention to prevent hospital admissions.
