Job Title: Senior LTSS Service Care Manager (RN)

Location: McAllen, TX area (Field-based with remote administrative work)

Type: 6-Month Contract

Start Date: April 21, 2025

Schedule: Monday-Friday, 8am-5pm CST (OT may be required based on caseload)

Travel: Up to 2-member home visits per day, 3 days a week. Occasional travel to Corpus Christi (2x/month)

About the Role

We are seeking a Senior LTSS Service Care Manager (RN) to join our STAR Kids service coordination team. This is a field-based position focused on children with special needs receiving IDD waiver services (CLASS, DBMD, TxHmL, and HCS waivers). You will coordinate care, complete assessments, and work closely with interdisciplinary teams and external providers to ensure comprehensive service delivery.

What You'll Do

  • Perform member assessments both telephonically and in-home
  • Coordinate care with providers including private duty nursing, DME suppliers, and pharmacies
  • Collaborate with IDD waiver case managers to meet the comprehensive needs of members
  • Maintain a caseload of 65 members with monthly telephonic and quarterly field visits
  • Document all activities per Centene policies and regulatory guidelines
  • Work independently while maintaining consistent communication with remote team

Who You Are

  • Licensed RN (Required)
  • Associate's Degree in Nursing required; BSN preferred
  • Bilingual (English/Spanish) strongly preferred
  • Minimum 2 years of clinical nursing in acute, clinical, or community settings
  • 1+ years of case management in managed care setting
  • Comfortable using Microsoft Office and navigating healthcare systems
  • Experienced with medical decision tools such as InterQual or NCCN
  • Strong self-starter with high accountability and member-first mindset
