Job Description Summary:The purpose of the SCAN-Providence St. Joseph Chapman University Post-Graduate Year 1 Managed Care Residency Program is to develop managed care pharmacist clinicians with diverse senior patient care, leadership and education skills who are eligible for board certification and postgraduate year two (PGY2) pharmacy residency training.
The resident will gain experience in pharmacy management areas such as: formulary management, utilization management, health plan management, P&T Committee meetings, Interdisciplinary Team meetings, coverage determinations, appeals, grievances, pharmacy administration, clinical programs development and implementation, medication therapy management, and community ambulatory care. In addition, the resident will learn about Medicare Part D regulations, STAR and HEDIS quality measures, pharmacy trends analysis, and community ambulatory care.
The residency is designed to provide experiences that will achieve the four core managed care residency competencies: patient care, advancing practice and improving senior patient care, leadership and management, teaching, education, and dissemination of knowledge. Patient care will be accomplished through a longitudinal primary care experience at Providence St. Joseph Health, a large, diverse medical group in Orange County.
Responsibilities:Program Experiences:
- Formulary management, utilization management, medication therapy management and health plan management
- Pharmacy and Therapeutics (P&T) Committees and interdisciplinary teams
- Coverage determinations, appeals and grievances
- Pharmacy administration, trends and analysis
- Managed care clinical programs development and implementation
- Quality measures, such as HEDIS and Medicare 5-Star Medicare Part D regulations
Teaching skills professional development program Accountable Care Organization (ACO)/ Patient-Centered Medical Home (PCMH) Chronic disease management Population health Specific Tasks:
- Provide clinical pharmacist services within SCAN Health Plan Provide direct ambulatory and population-based patient care at Providence St. Joseph Heritage Health
- Earn a teaching skills certificate from Chapman University
- Conduct a yearlong outcomes-based research project
Required Qualifications:- A Doctor of Pharmacy Degree from a School of Pharmacy accredited by the Accreditation Council for Pharmacy Education (ACPE)
- A pharmacist license, valid in California (or are capable of obtaining licensure in the state)
- A current GPA of at least 3.0 on the standardized GPA scale, or a "Pass" grade in a "Pass/Fail" program
- Strong communication and interpersonal skills and a commitment to teamwork, as well as the ability to interact with others in various environments
- The ability to provide clinical and classroom instruction to student pharmacists
- A completed application, submitted along with three letters of recommendation, current pharmacy college transcripts, a letter of intent and your Curriculum Vitae
