Brief Overview
Acts as a resident advocate, promoting the welfare and dignity of all residents.
Ensures that documentation supporting resident care is maintained and updated as required.
Facilitates the care planning process for residents within the guidelines of state regulations.
Recognizes that families are an important support for resident care and involves them in the overall plan of care.
Communicates effectively with family members regarding resident's condition, preferences and needs and addresses concerns in a respectful manner.
Monitors resident's condition, appropriately responds to changes in condition; participates in the delivery of quality care and services.
Coordinates resident care and services using sound managerial and clinical judgment, appropriate objectives and high standards of care and customer service.
Maintains effective communication with staff, peers and administrative personnel and with particular attention focused on neighborhood teams.
