What You Will Do:- Performs home care visits after the initial assessment, collects on going data, and reports to the RN Clinical Supervisor, implementing the POC.
- Provides skilled nursing care in the patients home as ordered by the attending physician in conjunction with the Plan of Care.
- Autonomy within your position, use your training, critical thinking and knowledge of your patients to make decisions
- Submits clear and appropriate documentation in a timely manner and documents patient education.
- Communicates effectively to physicians, supervisors, and co-workers in a timely manner.
- Promotes continuity of care with appropriate admissions, transfers, and charges.
- Other duties as assigned.
Skills:- Effective communication skills (verbal and written)
- Experience with Electronic Medical Records (EMR) software is preferred
- Time management and Organizational skills
- Demonstrated ability to work autonomously as a registered nurse
- Excellent planning, organizing and communicating.
What you will need:- Graduate of an accredited school of professional nursing, BSN preferred, currently licensed as an RN through the Ohio State Board of Nursing.
- Prior Nursing experience in Hospice or Home Health setting preferred. At least one year's medical/surgical nursing experience preferred.
- BLS/CPR Certification required
- Must possess a valid State of Ohio motor vehicle operator's license and be insurable under Firelands' auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.
- Independent decision-making skills and abilities to render nursing care in an independent setting.
- Ability to rotate "On Call" after hours, weekends and holidays.
