As an RN Patient Care Coordinator, you will be essential in delivering high-quality patient care and managing cases effectively. Reporting to the nursing supervisor, your role will involve utilizing core skills such as patient assessment, catheterization, and IV insertion to ensure optimal patient outcomes. Your premium skills in Epic and critical care experience will enhance our care delivery, while relevant skills in health coaching and community care will support holistic patient management. Responsibilities
Coordinate patient care activities and manage case management processes to ensure optimal patient outcomes.
Conduct thorough patient assessments and maintain accurate records using EHR systems.
Administer IVs, perform catheterizations, and provide tube feeding as needed.
Collaborate with healthcare teams to develop and implement individualized care plans.
Educate patients and families on health management and treatment options.
Ensure compliance with nursing standards and maintain an active RN license.
