The MDS / Care Plan Coordinator ensures timely and accurate resident assessments and interdisciplinary care plans in compliance with Federal and State guidelines. Key responsibilities include:
Integrating nursing, dietary, social, and rehabilitation services for proper assessment and reimbursement. Completing Medicare/Medicaid case-mix documents for appropriate reimbursement. Conducting ongoing MDS reviews to achieve optimal Resource Utilization Group (RUG) categories. Overseeing MDS/PPS documentation and submission processes. Preferred qualifications: Registered Nurse with state licensure and long-term care experience.
