New Mercer Commons Assisted Living is located at 900 Centre Ave. Fort Collins, CO and serves a wide range of needs by offering three unique sections of care: Assisted, Assisted Plus, and Memory Care all located within one building. We cater to individualized needs by offering seven styles of apartments ranging from Studios to Royal Suites. Compensation: Wages start at $23.00-$36.00/hr. depending on experience and licensure Shift and weekend differential pay Opportunities for growth within our company Benefits: Our employees who work 24+ hours per week qualify for a full range of benefits including Health Insurance Vision Insurance Dental Insurance Free health care clinics for anyone covered by the healthcare plan 401(k) Paid Time Off (PTO) Discounts at our pharmacy Discounts at our medical equipment stores Work Schedule: Part-time 24+hours per week 11:00 am - 7:30 pm RN/LPN Conduct comprehensive assessments of residents' health status Develop and implement individualized care plans based on assessment findings Administer medications and treatments according to prescribed protocols Monitor residents' vital signs and health condition, and report any changes to the physician or supervisor Communicate effectively with residents, families, and interdisciplinary team members Coordinate care with physicians, therapists, and other healthcare providers Provide guidance and support to caregivers and other staff members Participate in care conferences to review resident care plans and progress Qualifications For RN: Bachelor's or Associate degree in Nursing (BSN or ADN) from an accredited school of nursing and currently licensed in the State of Colorado For LPN: Completion of a state-approved practical nursing program and current LPN license in the State of Colorado Current CPR certification Experience in geriatric care or assisted living preferred Strong clinical assessment and critical thinking skills Excellent communication and interpersonal skills Ability to work effectively as part of a team and independently Columbine Health Systems is proud to offer a full array of benefits.
