Job Summary:

The Clinical Coordinator plays a key leadership role. This position ensures the delivery of high-quality, patient-centered care by coordinating clinical staff, supporting regulatory compliance, and fostering communication between caregivers, patients, physicians, and administrative staff. This person is responsible for QAPI and Oasis, may do visits, and will be coaching staff.

Key Responsibilities:

  • Monitor patient care plans to ensure compliance with physicians' orders, patient needs, and agency policies.
  • Ensure accurate and timely electronic medical records (EMRs) documentation and compliance with Medicare, Medicaid, and state regulations.
  • Provide clinical guidance, performance feedback, and disciplinary actions when necessary.
  • Conducts regular case conferences and assists in developing individualized care plans.
  • Collaborate with physicians, case managers, and families to ensure continuity and care coordination.
  • Participate in quality assurance and performance improvement (QAPI) initiatives.
  • Maintain knowledge of current home healthcare standards and regulations.

Qualifications:

  • Registered Nurse (RN) license in good standing (required).
  • Minimum of 2-3 years of experience in home health care; supervisory or case management experience preferred.
  • Strong understanding of Medicare/Medicaid regulations, OASIS documentation, and EMR systems.
  • Excellent organizational, leadership, and communication skills.
  • Ability to multitask and make sound decisions in a fast-paced environment.
  • CPR certification and a valid driver's license with reliable transportation.

Preferred Qualifications:

  • Bachelor's degree in Nursing (BSN), or Licensed RN in Texas.
  • Familiarity with home health software; they use Netsmart or Devero.
