Job Description: Advanced Hospice RN Clinical Liaison

Position Overview:

Advanced Hospice is seeking an experienced and resilient RN Clinical Liaison to join our fast-growing team. This role is essential in providing high-quality hospice care by ensuring seamless communication and education among patients, families, and healthcare facilities.

Location :

Pittsburgh, PA

Schedule :

Monday to Friday with some rotating on-call responsibilities.

Travel Requirements:

This position requires 100% travel to patient homes, facilities, and hospitals as necessary.

Responsibilities:

Conduct hospice eligibility assessments based on thorough knowledge of Medicare Hospice Regulations.

Educate patients, families, and facility staff about hospice services and patient-specific care plans.

Act as a primary point of contact to ensure coordination and continuity of care.

Utilize strong communication and time management skills to effectively manage multiple cases and priorities.

Participate in interdisciplinary team meetings to review patient plans and compliance with hospice criteria.

Provide ongoing support and education on the hospice process to healthcare providers within the community.

Qualifications:

Must possess an active RN license.

Proven hospice experience with a deep understanding of Medicare regulations and eligibility requirements for hospice care.

Exceptional interpersonal, communication, and presentation skills.

Robust organizational and time management skills are essential.

Must be adaptable, patient-focused, and capable of handling challenging situations with tact and empathy.
