Position Summary:

Our Client is seeking a RN Case Manager who will be performing comprehensive utilization reviews and proactive discharge planning for inpatient admissions. The RN Case Manager plays a critical role in guiding care transitions, working directly with patients and families to develop post-acute care plans that address both immediate and long-term needs.

Key Responsibilities:
Performs utilization review and discharge planning to inpatient admissions.
Communicates with attending and house staff when options to inpatient care are identified.
Utilizes clinical expertise to determine the appropriate level of care for patients. Leads the healthcare team in selecting appropriate, safe, hospital or alternative care options.
Works with nursing and social work staff to coordinate and facilitate patient discharge.
Works with patients and families to facilitate post hospital care and develop plans to meet short and long-term needs.
Accurately manages observation status patients by working with physicians to ensure appropriate utilization.

Required Qualifications:
Active MA RN License
AHA BLS.
COVID Vaccines + Booster.
Three years of medical/surgical nursing care experience. Experience in Case management required.

Preferred Qualifications:
EPIC experience preferred.
Bachelor's degree preferred.
