RN Case Manager

The RN Case Manager provides compassionate in-home care for hospice patients, coordinating with physicians and teams to ensure comfort and quality of life.
•  Responsibilities include assessments, care planning, patient and family education, and maintaining accurate records.
•   Respond promptly to calls, visits, and after-hours admissions or deaths as needed.
•   Build communication with families, physicians, and staff, addressing physical, psychological, social, and spiritual needs.
•   Instruct caregivers on medication, personal care, safety, and preparing for end-of-life events.
•   Attend team meetings, evaluate hospice aides and LPNs regularly, and develop hospice philosophy understanding.
•   Prior hospice, home health, or geriatrics experience preferred; a $3,000 sign-on bonus is offered.
•   Provides 24/7 coverage and supports families during death, ensuring compassionate and comprehensive hospice care.
