RN Case Manager

Provides case management for assigned patient populations, promoting cost-effective care and interdisciplinary collaboration. Coordinates care, performs patient assessments, and ensures appropriate referrals and documentation. Advocates for patients, participates in performance improvement initiatives, and collaborates with Utilization Review Nurse. Requires a Registered Nurse with a Bachelor's degree or 5 years of hospital Case Management experience. Must have 3 years of recent clinical healthcare experience and current licensure in Virginia. Other qualifications include effective communication skills, computer literacy, and knowledge of community resources.
