Pay Rate: $3,300/week (all-inclusive)

Job Description:

We are seeking a highly skilled and compassionate Registered Nurse (RN) - Case Manager to join our care team in Plymouth, MA for a 13-week contract assignment . As an RN Case Manager, you will be responsible for coordinating care plans, managing transitions, and advocating for patients throughout their treatment journey. This role is ideal for a nurse who thrives in a collaborative environment and is committed to delivering patient-centered care.

Key Responsibilities:
Assess patient needs and develop individualized care plans
Coordinate interdisciplinary care teams to ensure efficient care delivery
Facilitate discharge planning and post-discharge support
Act as a liaison between patients, families, and healthcare providers
Monitor patient progress and adjust care plans as needed
Maintain accurate and timely documentation in compliance with facility policies and regulations

Requirements:
Active Registered Nurse (RN) license in Massachusetts (or compact license, if applicable)
Minimum of 2 years of RN Case Management experience
Strong knowledge of utilization review, discharge planning, and patient advocacy
Proficient with EMR systems and case management software
BLS Certification (American Heart Association)

Contract Details:
Assignment Length: 13 weeks
Start Date: 06/16/2025
End Date: 09/13/2025
Schedule: Monday to Friday, 8-hour day shifts
Compensation: $3,300 per week (travel/lodging stipend included if applicable)
