RN Case Manager

This role involves developing, implementing, and evaluating patient care plans across the health continuum, collaborating with providers, teams, and external partners to advocate for patients and caregivers.

•   Conducts comprehensive assessments addressing clinical, social, and economic factors impacting health management.

•   Identifies barriers, risks, and resource needs to ensure safe transitions and care coordination.

•   Coordinates clinical care, communicates effectively with interdisciplinary teams, providers, payers, and post-acute partners.

•   Educates patients, families, and providers on care practices, documentation, resource utilization, and transition planning.

•   Maintains professional development and adheres to institutional policies, with required licensure and certifications.
