RN Case Manager

Seeking a full-time RN Case Manager in Lafayette, IN to join an expanding home health care team, balancing office and fieldwork.

•  Lead care planning, coordinate with healthcare providers, monitor patient progress, and educate patients and families.

•  Conduct assessments, develop and revise care plans, and supervise other health personnel.

•  Require RN licensure, 1+ year nursing experience, comfort with Pediatrics and Geriatrics, and current CPR certification.
