In this position, every day brings new and energizing challenges. You are at the center of the patient healthcare continuum, from admission to discharge. You act as liaison to patient and family, physicians and external agencies, and the entire care team. Your best day is when you and your team members discharge patients knowing you delivered compassionate, quality and efficient care. With a team of 150 Case Management team members the average Case Management nurse to patient ratios are 1:25. A keen eye for detail comes naturally to you as does thorough and timely documentation and reporting. You quickly grasp applicable regulations and standards, with the assurance to handle difficult situations on your own, or to escalate them as appropriate. You employ the political savvy to collaborate in a large, fast-paced, and ever changing system.

Qualifications:

Experience and education minimum requirements

Bachelor's degree in Nursing
One year acute care hospital experience.
Three years of acute care hospital experience may be substituted in lieu of education.

Licenses and certifications minimum requirements

Current California Registered Nurse license.
BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA)
