POSITION SUMMARY - RN Case Manager

POSITION DUTIES - Progression of care; discharge planning; Multidisciplinary rounds; Discussions about dc plans with families and MDs.

MINIMUM REQUIRED QUALIFICATIONS - TX/compact RN license; BLS; Bedside CM experience 1 year minimum; Post acute care knowledge; Interqual, Meditech, MIDAS experience.

PREFERRED QUALIFICATIONS - Knowledge of throughput in acute care setting.

LENGTH OF ASSIGNMENT - 13 weeks

SHIFT / HOURS PER WEEK - 7:30A-4:00P, (Friday weekend rotate), no call required.

SYSTEMS - Meditech/Midas/Interqual

START DATE - ASAP
