Current Colorado RN License or compact covering Colorado Recent college grads welcome, will train Valid Driver's License Weekday availability 8AM - 5PM Weekend availability with rotating on-call responsibility Summary:
The Registered Nurse Case Manager promotes quality service to clients in their homes, serving as a liaison for patient care coordination. Responsibilities include patient admission, care plan development, continuous assessment, and communication with healthcare teams. The role requires participation in on-call duties, quality management, and supervision of LPNs and aides. Knowledge of home health regulations is essential. The position is part-time with a pay range of $45.00 - $90.00 per hour, offering benefits like 401(k) and paid sick time.
Work location: On the road, with a requirement to relocate to Cañon City, CO before starting work.
