PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS:
Assesses patient's health problems and needs and develops plan of care.
Provides direct skilled nursing care as per physician orders.
Observes patients, charting and reporting changes in patient's conditions, such as adverse reactions to medications or treatment and taking any necessary action.
Document medical records and prepare other reports as needed.
Collaborates with the physician and other home health care providers, within and outside agency to enhance patient care.
SUPERVISORY RESPONSIBILITIES
Completes home health aide supervisions and orientations as needed.
Co-signs orders and provides oversight of care for LPNs.
QUALIFICATIONS:
Graduate of an accredited school of nursing - required.
Current license to practice as a registered nurse in the state of Massachusetts - required.
Current CPR - required.
One year nursing experience, preferably med-surg - preferred
Possess a valid State of Residence Driver's license and reliable transportation - required.
