Job Description: Responsible for coordinating and negotiating patient care services, reviewing medical necessity for admissions, and providing discharge planning. Key tasks include:
Applying medical necessity criteria to patient records. Documenting discharge planning referrals and coordinating post-acute care placements. Educating care team members on level of care criteria. Implementing strategies to reduce length of stay. Monitoring care plan effectiveness and intervening as needed. Qualifications: Bachelor's degree in Nursing with 3 years of experience or Associate's degree with 5 years required. Must be licensed as an RN in Alabama and certified in BLS. Additional certifications may be required based on the unit.
