Job Description: Provides patient service coordination for the Rehabilitation Unit with the goal of assisting the patient with the transition to the next level of care by coordinating the ongoing services with the patient, family, team and payer.
Education:
Experience:
Experience in Discharge Planning, Utilization Review and/or Case Management - Required
Experience in Acute Rehabilitation with knowledge of regulations - Preferred
Certifications/Licensures:
Skills:
Excellent patient clinical review skills and reporting to external case managers as required.
Excellent customer service and time management and organizational skills required.
Clear, through and concise documentation skills including attention to detail, proactive analysis and team collaboration.
Strong written and verbal communications skills.
Knowledgeable about Medicare and commercial contracts for acute and post-acute care.
Proficient in working between various IT systems.
Work Shift: 08.0 Per Diem No Waive (United States of America)
Pay Range:
$83.82 - $113.15 Hourly
Offer amounts are based on demonstrated/relevant experience and/or licensure. Pay will be adjusted to the local market if hired outside of the Bay Area .
Note: Positions at JMH which are exempt (not eligible for overtime) under the level of Manager are listed as hourly for compensation purposes on this posting. The work shift will contain the word 'exempt' on it.
