Job Description:
Intermountain Healthcare's Population Health Management Organization, has primary accountability for the performance of risk-based products and operational accountability for the clinically integrated network and senior services. Translating the value-based strategy into operations is core to the daily work and focuses on all at-risk programs and payers (e.g. Medicare ACO, Medicare Bundles, Medicaid and Medicare Advantage, Intermountain Employee Health Plan, SelectHealth commercial products). The post-acute care manager works collaboratively with physicians and other members of the healthcare team to improve the health of patients with chronic conditions or complex needs. The position educates patients and caregivers to help manage their healthcare needs. The care manager facilitates communication, coordinates services, addresses barriers, and promotes optimal allocation of resources while balancing clinical quality and cost management. The care manager leads daily work with partners and patients in post-acute care to improve Intermountain client performance in post-acute services in at-risk contracts.

This role supports the PACM program in the Salt Lake county area in the current SNFs. SNFs supported is subject to change pending business needs: Copper Ridge, St Joseph's Villa, City Creek Post Acute, Legacy Village Rehab and Monument Healthcare Murray Creek.
Job Essentials
- Participates in post-acute weekly utilization meetings with facilities to ensure patients in at-risk contracts are being cared for in adherence to Intermountain clinical pathways. Assesses and escalates medical management challenges to post-acute care providers (physicians and advanced practice providers).
- Engages in patient and family care conferences in skilled nursing facilities (SNF) to assist in identifying potential barriers to discharge home and set expectations regarding expected length of stay. Coordinates with care providers and leaders to reach optimal solutions.
- Leads and ensures patient safe and seamless transition from SNF to home (e.g., coordination of return to clinic appointment, coordination of necessary home services, handover to outpatient nurse care manager, provide patient and family caregiver education).
- Ensures a plan of care is developed and maintained for members receiving services using established evidence-based care process models or care plans.
- Uses knowledge of patient's benefits and resources, directs and assists patients to appropriate healthcare resources and services, and implements other health management strategies.
- Leads post-acute quality improvement with post-acute partners to improve patient care across the continuum.
- Evaluates post-acute facilities and services for development and continued engagement in the post-acute network.
- Communicates and shares the vision for post-acute services in at-risk contracts to key internal and external stakeholders.
- Identifies, analyzes, and monitors industry, regulatory, technology and market-based trends that impact post-acute services. Provides value-based program education to post-acute providers.
Minimum Qualifications
- RNs hired or promoted into this role need to have or obtain their BSN within three years of hire or promotion.
- Current RN license for state in which the nurse practices.
- Five years of clinical nursing experience.
- Experience in post-acute care setting.
- Demonstrated effectiveness in a matrix organization.
Preferred Qualifications
- Bachelor's degree in Nursing (BSN). Education must be obtained through an accredited institution. Degree will be verified.
- Case management certification.
- Experience in care transformation or working with at-risk payment models.
- Understanding of healthcare market dynamics in Utah.
- Experience working independently while remaining motivated, flexible, and positive in a rapidly changing environment.
Physical Requirements:
- Ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and be able to assess patient needs.
- Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, etc.
