This RN Care Manager role offers up to a $5k hiring incentive and focuses on coordinating patient care in collaboration with families, physicians, and care providers. Responsibilities include utilization review, discharge planning, documentation, and patient advocacy across the continuum of care. Must be a CA-licensed RN with BLS (AHA), and three years of clinical nursing experience (preferred). BSN, CPHQ or CCM certification preferred. Strong organizational, communication, and independent work skills required. Physical demands include frequent movement and occasional lifting up to 50 lbs.
