Provide full-time triage nurse responsibilities in a call center environment, serving an urban community health center patient
population, in accordance with the policies and procedures and philosophy of PCHC. All duties/responsibilities are
performed across life cycle (infants, children, adolescents, and adults).
Duties & Responsibilities: - Receive, triage and respond to incoming telephone calls from patients seeking clinical direction and advice.
- Uses standardized adult and pediatric protocols. Seeks assistance when a protocol is not available or when clinically appropriate.
- Quickly and accurately assesses patient's needs by actively listening to caller and asking proactive questions.
- Directs patient to the appropriate level of care. When appropriate, ensures the patient is correctly scheduled with a PCHC provider.
- Identifies emergent situations and effectively triages to higher level of care.
- Makes clinically appropriate recommendations for action/intervention and issue resolution.
Qualifications: - Licensed Registered Nurse (RN) in good standing without restriction in the state of Rhode Island. Must have excellent assessment, critical thinking, and communication and customer service skills.
- Minimum of 5 years nursing experience in an emergency room setting, primary care, or urgent care clinic.
- Work experience as a community health nurse preferred.
- Strong nursing assessment skills and level of comfort working in a fast paced medical environment, both independently and as a member of the medical team.
- Experience in Triage, or similar ability to rapidly use the nursing process to assess, diagnose, plan, implement and evaluate in real-time situations.
- Comfortable communicating with patients with the use of interpreters.
- Spanish speaking strongly preferred.
Education: - Bachelor's degree preferred or equivalent work experience.
- BLS Certification required.
