Plaza de la Raza Child Development Services, Inc. (Plaza) operates a comprehensive Head Start program serving children from birth to five years of age. Plaza is currently accepting bids for a Behavior Intervention Consultant to support the disabilities and mental health services offered to children and families. Services to be provided from July 1, 2025 - June 30, 2026. Part-time hours to be negotiated.
Job Description
Scope of Work
• Review, evaluate, and interpret child health and nutrition screening forms to identify and address health concerns.
• Assist with medication monitoring and conduct medication reviews.
• Document all reviews directly in the database system, ensuring accuracy, timeliness, and completeness in alignment with the documentation standards required.
• Complete and or review Individualized Health Care Plans for children as needed.
• Collaborate with staff on the completion of Individualized Nutrition Health Care Plans.
• Provide staff training on medication administration
• Conduct follow-up and provide education to parents of children with identified health conditions or missing information.
• Monitor medication uses and assist with regular medication reviews.
• Attend Health and Mental Health Advisory Meetings.
• Participate in Multidisciplinary Team Meetings
• Perform other health-related duties in compliance with Plaza's policies and procedures as agreed.
Desired Skills and Experience
Requirements and Specifications
• Registered Nurse License/Certificate by California Board of Registered Nursing.
• Three (3) years of experience in health services, preferably in pediatrics or maternal/child health.
• Clearance from the State of California, FBI, and Child Abuse Central Index
• Documentation of TB clearance, Tdap, MMR, and Influenza Immunizations (or flu vaccine waiver).
• Verification of Liability Insurance
