This individual will be responsible for: Responsible for accepting incoming referral information via multimedia and working directly with bed management and direct care nursing to coordinate inpatient bed assignments (particularly critically ill) for patients at Emory Healthcare. Coordinates, prioritizes, and optimizes patient flow using knowledge of clinical and operational activities throughout the hospital network. Collect clinical information to determine patient's level of care while collaborating with providers to arrange acceptance by the attending EHC physician to the most appropriate EHC hospital. Monitor bed availability and coordinate bed assignments for all patients receiving care in an inpatient or outpatient bed and for patients being transferred between units and from outside facilities to maximize bed utilization. Communicate with referring hospital to coordinate transportation arrangements and provide an assigned bed. Communicate clinical information to transportation company, physicians, physician designees, referring facilities, and charge nurses. Ensure safe and timely placement of patients based on admission status, physician requested level of care, patient's clinical information and nursing units admission/discharge criteria. Maintain the patient transfer and flow software to accurately reflect physical location of all patients occupying an inpatient bed and/or ancillary holding area. Confirm patient insurance eligibility and obtain specific payor benefits. Obtain and confirm admission notifications and authorizations for external clinic/facility transfers. Document findings in the Financial Billing System Exhibit appropriate customer/guest relations in all interactions. Consistently communicate information in a timely, accurate and concise manner. Apprise Manager of significant developments affecting patient flow. Participate on Nursing and Hospital committees and councils as required. Perform other duties as assigned. MINIMUM QUALIFICATIONS: Bachelors degree in nursing required. Must have a valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board. Experience: Three years of recent (within the past five years) intensive care nursing experience that includes supervision and patient bed placement. Additional required skills: strong critical thinking, accurate assessment and problem solving, excellent verbal and written communication skills, attentiveness to detail, high-level analytical ability required in order to resolve complex patient flow issues, strong interpersonal skills, knowledge in nursing concepts, practices and procedures.
