Within the inpatient setting to assess and address patient and family needs related to effective and efficient movement through the hospital system and beyond. Perform initial assessment on all patients and begin discharge process from day one of patient's stay. Complete level of care assessment to ensure patients are receiving the appropriate level of care, and to ensure that the patient transitions through their stay without barriers or delays in care delivery. Maintain expert knowledge of Interqual Level of Care criteria. Work closely with the Admitting Physicians to ensure appropriate documentation of clinical information necessary to support or determine alternative level of care. Coordinate discharges home and facilitate transfers to external placement facilities

About the Role: RN Case Manager Inpatient $10,000 Sign on Bonus, $5,000 relocation available for qualified candidates

The Registered Nurse Case Manager at UVA Health is integral in coordinating patient care and ensuring a seamless healthcare experience. This role involves close collaboration with the healthcare team to develop and implement individualized patient care plans, focusing on promoting quality outcomes, managing costs, and ensuring continuity of care.


Key Responsibilities:
  • Patient Assessment: Conduct comprehensive assessments of patients' physical, psychological, and social needs.
  • Care Coordination: Collaborate with multidisciplinary teams to develop, implement, and evaluate care plans.
  • Patient Advocacy: Act as a liaison between patients, families, and healthcare providers to advocate for patients' needs and preferences.
  • Resource Management: Utilize resources efficiently to optimize patient outcomes and manage healthcare costs.
  • Discharge Planning: Develop and execute effective discharge plans, ensuring patients and families are prepared for the transition from hospital to home or other care facilities.
  • Documentation: Maintain accurate and timely documentation of all case management activities.
Skills Needed
  • Clinical Expertise: In-depth knowledge of nursing principles, practices, and procedures.
  • Critical Thinking: Strong analytical skills to assess patient needs and develop appropriate care plans.
  • Problem-Solving: Ability to identify and resolve complex issues related to patient care and resource management.
  • Team Collaboration: Ability to work collaboratively within a multidisciplinary team to achieve patient care goals.
Position Compensation Range: $81,328.00 - $108,035.20 Annual

MINIMUM REQUIREMENTS
Education: Graduate of accredited nursing program, Bachelor of Science in Nursing (BSN) Strongly Preferred, or required within 5 years of hire.
Experience: 3 years Relevant Experience
Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. Accredited Case Manager (ACM) Certification within 3 years of hire.

PHYSICAL DEMANDS
Job requires standing for prolonged periods, frequently traveling, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
